Dr. Thomas Lee Horowitz, dedicated physician, mentor, public servant, loyal friend, a loving father, and devoted husband, passed away on July 6th at the age of 75.

Dr. Thomas L. Horowitz, is survived by his beloved wife, Peggy Ann Horowitz; his loving children, Frederick Maxwell Horowitz and Anne Louise Horowitz; his mother, Barbara Jean  Horowitz; his sister, Marjorie Beth Bonar; his nephew, David Alan Bonar; and many cherished friends, colleagues, patients, and members of the medical community whose lives he enriched through his kindness, wisdom, unwavering dedication, and humor.

He was preceded in death by his father, Fred Gerton Horowitz, M.D., and his brother, Michael Bernard Horowitz.

For more than four decades, Dr. Horowitz dedicated his life to the practice of medicine, caring for countless patients with exceptional skill, compassion, and integrity. His unwavering commitment to improving healthcare extended far beyond his private practice, leaving a lasting impact on hospitals, public health policy, medical education, and the broader Los Angeles community.

A proud alumnus of the University of Southern California, Dr. Horowitz earned degrees in Psychology and a Master of Public Administration. He was honored as a member of Skull and Dagger and USC Associates. He later earned his Doctor of Osteopathic Medicine degree from the College of Osteopathic Medicine of the Pacific in 1982, launching a distinguished career marked by leadership, service, and advocacy.

Throughout his medical career, Dr. Horowitz held numerous leadership positions at some of Los Angeles' most respected hospitals. At St. Vincent Medical Center, he served as Chair of Bioethics, former Chair of Family Practice, and a member of the Continuing Medical Education and Medical Executive Committees from 1984 through 2020. At Good Samaritan Hospital, he chaired the Department of Emergency Medicine and Family Practice and served on both the Medical Executive and Continuing Medical Education Committees. At Hollywood Presbyterian Medical Center, he chaired the Board of Governors and served as Bioethics Consultant, helping guide the development of a new patient care tower, emergency department, operating rooms, and innovative patient-centered hospital design.

During the height of the AIDS epidemic, Dr. Horowitz became a passionate advocate for education, prevention, and compassionate care. He served on the Task Force on AIDS Risk Reduction and Education from 1985 to 1987 before being appointed to the Los Angeles County Commission on AIDS, where he served from 1987 to 1996 and chaired the commission from 1991 to 1992.

His commitment to public service was well recognized. The city of Los Angeles appointed him as Medical Director for the Los Angeles Open Golf Tournament from 1987 through 1995. Then later as Medical Director of the Los Angeles–Guangzhou Medical Exchange Program from 1990 to 2020 and the Los Angeles–St. Petersburg Medical Exchange Program beginning in 1992. In these roles he fostered international collaboration among healthcare professionals expanding his impact internationally. 

A fervent advocate for healthcare policy, Dr. Horowitz devoted tireless hours to organized medicine. Through the Los Angeles County Medical Association, he chaired committees focused on Emergency Medical Services, AIDS, Care for the Medically Indigent, and the Public Health Reference Committee. In recognition of his extraordinary contributions to health policy and the relationship between government and the medical profession, he received the Los Angeles County Medical Association's Lifetime Achievement Award in 2017. He later served as Chair of the Los Angeles Medical Association Political Action Committee from 2020 to 2021 and was a longtime leader of OsteoPAC, joining the board in 2004 and serving as Chairman beginning in 2013.

Dr. Horowitz also helped shape healthcare delivery through his leadership with L.A. Care Health Plan, serving on its Executive Board from 2007 to 2016, chairing the Quality Compliance Committee from 2008 to 2010, and ultimately serving as Chairman of the Board of Governors from 2013 to 2016.

His spirit of adventure and service defined every aspect of his life. He was a proud member of the Adventurers Club, participating in deep-sea diving expeditions throughout Mexico. His lifelong dedication to law enforcement began as a teenage Explorer Scout with the Los Angeles Police Department and continued through his volunteer service with the Long Beach Police Department's "Doc Squad" Reserve Unit, where he became a POST Level III Peace Officer. He later served as Tactical Medicine Coordinator beginning in 2003 and Medical Corps Chair in 2007.

Dr. Horowitz was also a Master Mason and a Noble of Al Malaikah Shrine, reflecting his lifelong commitment to fellowship, charity, and service.

While his professional accomplishments were extraordinary, those who knew Dr. Horowitz will remember him most for his generosity, quiet wisdom, intellectual curiosity, sense of humor, and steadfast dedication to helping others. He mentored generations of physicians, advocated zealously for patients, and approached every challenge with compassion and conviction. His life was defined not by the many offices he held, but by the lives he touched through healing, leadership, and friendship.

A celebration of Dr. Horowitz's remarkable life will be held at Hillside Memorial Park on July 9th. In lieu of flowers, the family kindly requests that memorial donations be made to Shriners Hospital of Southern California, supporting the fraternity's longstanding commitment to philanthropy and charitable care for children. This tribute reflects Dr. Thomas L. Horowitz's lifelong dedication to service, fellowship, and helping others. 

Thomas L. Horowitz, D.O., leaves behind an enduring legacy of healing, leadership, advocacy, and compassion. His influence will continue to be felt through the institutions he strengthened, the policies he helped shape, the physicians he mentored, and the patients whose lives were made better by his care. He will be deeply missed and forever remembered.
